I had the same problem on my Gen 2. If you remove the dust cover you can watch the action. Dry fire, keeping the trigger depressed. Rack the bolt carrier and watch closely while releasing the trigger. I had to file a tiny bit of material from the underside of the hook part of the disconnector. Also I forget to mention this earlier but I also used some wd-40 penetrating oil to try help loosen and dissolve any lube/sealant holding the plate in. I let that penetrating oil sit over night.
